Students use various forms to make academic requests within the Mathematics Program such as petitioning a course, requesting to repeat a course, enrolling in independent study or computing internship, or participating in undergraduate research.

Mathematics 3rd+ Attempt Request

Use this form if you would like to petition any course in the Math major, the Math or Actuarial Science minors, or if you would like to petition to attempt a BMATH or STMATH-designated course for a third or more time.

Mathematics Course Equivalency Petition

Use this form to have a course from another institution reviewed as equivalent to a UW Bothell Math course.
How to apply: Make sure you have a complete syllabus, a well thought out and a developed persuasive statement (educational reasoning and how this will help your educational experience) and projects or related coursework that helps support your request.

Independent Study and Undergraduate Research: STMATH 498/499

STMATH 498/499 an opportunity for students to obtain college credit by participating in a course that is supervised by a full-time Math faculty member in which the student meets the faculty’s objective while participating in a faculty sponsored research project or independent study related to the field of mathematics.
