Circulating Equipment Terms of Use Agreement
Before anyone is authorized to utilize the circulating equipment service, they must sign and submit a digital Terms of Use Agreement online. On this page you will find a direct copy of the Terms of Use Agreement that can be signed through an online program contracted through the University of Washington called DocuSign.

Terms of Use Agreement
Welcome to the University of Washington Bothell Information Technologies (UWB IT) Helpdesk Equipment Circulation Service. The following Terms of Use apply when you use this service. Please read them carefully before signing. A copy and confirmation of the agreement will be sent to the email you provide. If you do not agree to these Terms of Use, you may not access or use this service. UWB IT reserves the right to modify these Terms of Use as needed and will notify all users should changes be made.
About the service
The UWB IT Helpdesk Equipment Circulation Service allows eligible users to check out state-owned equipment for academic or professional university use free of charge for a limited loan period. Personal use is not permitted.

Eligibility and user responsibilities
You must be a current University of Washington Bothell student, faculty, or staff member to utilize this service. University of Washington students from the Tacoma or Seattle campuses must be enrolled in a course at the UW Bothell campus at the time of check out to be eligible to utilize this service.
A valid Husky card is required to check out equipment. You may not check out equipment using another person’s Husky card.
User and conduct restrictions
By using this service, you agree to the following:
- Inspect all equipment at the time of check out and report any defects or missing pieces you find to the technician processing your check out. Upon check in, you may be responsible for missing parts or damage to the equipment and your ability to continue to checkout equipment may be impacted.
- Call, email, or come in person to the UWB-IT Helpdesk to request equipment renewal.
- All renewal requests are approved or denied based on current equipment inventory. Once equipment has become overdue, it will no longer be eligible to be renewed and must be returned immediately.
- Report any damage, loss, or theft of equipment checked out to you.
- Clear all equipment memory of data before returning the equipment to the UWB-IT Helpdesk.
- All data stored on laptops and removable media is erased upon return. UWB-IT is not responsible for the loss of data left on a device.
- Return equipment to the UWB IT Helpdesk with all included accessories by 4:00pm on the due date. If equipment is returned with missing components, it will be considered a partial return. See the Late and Partial Returns section below for more information.
- If you’re unable to return your equipment in person, please contact us to explore alternative arrangements. Equipment circulation is not available during interim breaks. All circulating equipment needs to be returned by the end of each academic quarter.
- Comply with the rules, procedures, policies, and standards of conduct of the University of Washington.
Prior to using this service, all users must have read, digitally signed, and submitted the Circulating Equipment Terms of Use Agreement (this document) to the UW Bothell ITHelpdesk. The UW Bothell IT Helpdesk only accepts digital submissions. Paper submissions will not be accepted.
Late and partial returns
Faculty and Staff
You or your school/unit are responsible for any unreturned equipment.
Students
Late and partial returns may result in temporary or permanent restrictions on borrowing privileges.
- Partial Returns: If equipment is returned with missing components, it will be recorded as a partial return and considered an active loan until all components are received. Borrowing privileges may be restricted until all items are returned.
- Late Returns: If all checked out equipment is returned within 7 days of due date, borrowing may resume. If equipment is returned after 7 days, borrowing privileges will be suspended for 2 weeks.
Equipment Recovery Process
If there is no response to return reminders, the Equipment Recovery process will begin. Continued lack of response and not returning the equipment may result in a referral to Student Conduct.
Multiple policy infractions may result in permanent suspension of borrowing privileges.
Additionally, failure to comply with UWB IT notifications and/or policies is a violation of the UW Student Conduct Code under WAC 478-121-130 and is subject to reporting to the office of Student Conduct – Student Affairs.
All student users will be strictly held to the Student Conduct Code for the University of Washington, which can be found here:
Chapter 478-121 WAC: Violations of the Student Conduct Code (including but not limited to: abuse of others, acts of dishonesty, failure to comply, harassment or bullying, vandalism, and violation of law) may result in refusal of service, restriction from the program and/or reporting to the office of Student Conduct – Student Affairs.
