Students must complete the student information portion of the Measles Immunity Verification form and provide either: Attach copies of original immunization records; OR Attach a statement on letterhead stationery from their health care provider as proof of:
A. Date of two live virus measles (rubeola) vaccinations, both given after January 1, 1968. Vaccines must have been given no earlier than 12 months of age (on or after first birthday) and at least 28 days between doses. Please submit copies of the vaccinations with the UWB Measles Immunity Requirement Form.
B. Documented positive measles titer (rubeola blood test) lab result. Please submit copy of lab result with the UW Measles Immunity Requirement Form.
C. Documented history of measles (rubeola) disease. Please submit copy of medical records or letter from healthcare provider confirming disease with the UW Measles Immunity Requirement Form.

In the event of a measles outbreak on the campus, students who have not previously submitted proof of immunity to measles will be barred from classes, living groups, and other campus activities until health officials determine that there is no risk of them transmitting the disease to others.
Students who have questions or concerns about the measles immunity requirement are encouraged to contact the UW Bothell Office of the Registrar well in advance of their registration date.
