Student Forms & Resources


This page contains links for all Physical Sciences forms and handouts.

Many forms are implemented to Google Forms. To access Google Forms, you will need to log in using your UW email address (including the “” part) and password. If Google does not recognize your account, then you must activate it by doing the following:

  1. Log into MyUW​.
  2. Scroll down to UW Resources and select Accounts, Identity, and Email.
  3. Under Email, and Accounts and Identity, select Email Forwarding.
  4. Select UW G Suite on the left side of the page.
  5. Under UW G Suite Account Information next to Account Status, click the Turn On link.

Application Forms

  • Transfer Application Form. Use this form if you are a transfer student applying directly to the UW Bothell Chemistry or Physics program. UW Seattle and Tacoma students, please use this form.
  • Internal Application Form. Use this form if you are currently at UW Bothell as a pre-major student or are already enrolled in another UW Bothell program.

Registration / Sign-up Forms

  • BCHEM and BPHYS 498/499 Registration Request. Once you have discussed your research or independent study plans with your instructor, fill out this form to request registration for the credit. Please allow two weeks for processing. 
  • Capstone Presentation Sign-up. Use this form to sign up for a research symposium event (always on Friday of finals week each quarter).
  • Chemistry Student Mailing List or Physics Student Mailing List Subscribe/Unsubscribe. Subscribe to the Chemistry or Physics Student Mailing List to receive advising reminders and announcements, as well as information on events, courses, research, internships, jobs, scholarships, clubs, and other resources. Most messages are sent as a concise weekly UWB Sciences Digest.
  • Course Repeat RequestIf you have taken a course twice or more, you must complete this form to request registration for that course.

Petition Forms

Use these forms when you want to use a transfer course or other non-standard option for a major or minor requirement. Substitutions are considered on a case-by-case basis. You must submit supporting documentation such as a syllabus, a detailed letter from your instructor/supervisor, or a similar document that thoroughly explains the content of the course or experience. Please allow three weeks for processing.

These petitions are for degree-specific requirements. For university-level requirements such as General Education or Area of Knowledge, please contact your advisor directly instead of submitting a petition.
