Academic Breadth

K-8 teachers must show competence across a breadth of subject areas. Academic Breadth courses must be listed to verify completion of Elementary Education content area. A form is available on the School of Educational Studies website. To learn more about academic breadth, click here.

Advanced Placement Official Score

An official copy of your AP Score Report is required in order to transfer credits to your UW records. To learn more about advanced placement scores, click here.

Apply for Housing

If you are interested in living on-campus, an application for Housing is required. Applications typically open at the end of February. You may apply to housing here.

*Housing is not a requirement for enrollment at UW Bothell.

Complete I-20 Request

This form will help us collect the required information to start processing your UW Bothell I-20. You must provide foreign address information for your permanent address. To learn more about the I-20 form, click here.

Contact Your Counselor

Contact your admissions counselor about action required for your application. You may also call the Admissions Office at (425) 352-5000 or email us at uwbinfo@uw.edu to be directed to your admissions counselor.

Design Portfolio URL

English Proficiency Requirement

Proof of English language proficiency is required for all students who did not complete most of their primary and secondary education in the United States, Australia, Canada, Ireland, New Zealand, or the United Kingdom. For more information regarding English language proficiency, click here.

English Translation of Transcript

Translation is required for an original international transcript when the official transcript is in a language other than English.

F1 Visa Stamp

The F1 visa stamp is found in your passport. An expired stamp or one with another school’s name on it is acceptable. An example of an F1 visa stamp can be found here.

Form: Experience Working with Children

30 hours of work with children in a public elementary classroom must be documented. Forms are available on the School of Educational Studies website.

High School Transcript

An official academic record of the coursework you completed and the grades/number of credits you received for each class. To learn more about our transcript policies and how to send us your transcripts, click here.

Home School Transcript

A homeschool transcript includes course titles of each subject studied, duration of study, a short description of content, and grade or assessment of performance. To learn more about our transcript policies and how to send us your transcripts, click here.

I-94 Arrival Record

Print this AFTER your most recent arrival in the United States. Do not print your Travel History page. An example of an I-94 arrival record can be found here.

Immunization Requirements

Proof of immunity to Measles, Mumps, and Meningitis are required for enrollment. Please refer to emails sent from immunity@uw.edu or contact them directly. For more information about what is required, please go to the UW Immunization Requirement page.

International Credential Evaluation

An official evaluation by an approved translation & evaluation company must be provided for all post-secondary international transcripts. To learn more about international credential evaluation, click here.

K8 Essay Responses

Those applying to the Elementary Education Teacher Certification Program must answer the essay questions outlined on the School of Educational Studies website.

Letter of Recommendation – Academic

Elementary Education Teacher Certification only. A letter of recommendation from a professor or instructor is required. To learn more about the Elementary Education letters of recommendation, click here.

Letter of Recommendation – Employer

Elementary Education Teacher Certification only. A professional letter of recommendation from a supervisor is required. To learn more about the Elementary Education letters of recommendation, click here.

Letter of Recommendation – School Based

Elementary Education Teacher Certification only. A letter of recommendation from a school official who has observed your work with children is required. To learn more about the Elementary Education letters of recommendation, click here.

Mid-Year Transcript

Midyear transcripts are unofficial or official transcripts, which report your first term of senior year final grades. The first term will depend on the academic calendar system (semester, trimester, or quarters) your school is on:

  • Semester system – A midyear transcript reporting your first semester final grades from your senior year.
  • Trimester system – A midyear transcript reporting your final first trimester grades from your senior year.
  • Quarter system – A midyear transcript reporting your final fall quarter grades from your senior year.

The midyear transcripts provide the admission committee your most recent academic performance and are required to begin the review process of your application. You may upload an unofficial transcript to your student status page. If you are unsure how to access your student status page, search for the email titled “Thank You for Applying” to review the instructions for setting up your status page.

Military Transcript

Official military transcripts (Joint Services Transcripts) must be submitted to the Office of Admissions for evaluation. To learn more about our transcript policies and how to send us your transcripts, click here.

NCLEX Passage

Provide a record of your Washington RN License to show NCLEX passage.

Personal Statement

A personal statement of 450-600 words must be well constructed mechanically and thematically. It must demonstrate your drive and ability to study at UW Bothell and in your requested major, specifically. Transfer students should note that there may be major-specific prompts based on your requested major.

Resume

Required for students applying to Business or RN-BSN only. A resume is required to demonstrate experience (educational, work, volunteer) in the requested major.

RN-BSN Letter of Recommendation – 1

An academic recommendation is required from a nursing instructor. A 2nd professional recommendation can be substituted for those who graduated more than five years ago. To learn more about the letter of recommendation, click here.

RN-BSN Letter of Recommendation – 2

A professional recommendation is required from a current supervisor, preferably in health care. A clinical supervisor can serve as a professional reference for those just completing an ADN program. To learn more about the letter of recommendation, click here.

SAT or ACT Test Scores

The University of Washington Bothell no longer requires SAT or ACT scores (read the January 27, 2021 announcement for more information). Students who have taken the exams are welcome to send their scores, however, there is absolutely no advantage or disadvantage in doing so. Official test scores from SAT (school code: 4467) or ACT (school code: 4497) must be sent directly from the testing agency.

Setup Your MyUW netID and UW Email

Your UW NetID provides access to many online UW services, including MyUW and UW email. It ensures privacy of your personal information. Learn more about setting up your MyUW netID and UW email address.

Sign up to attend an Advising and Orientation Session

Advising & Orientation is the event where you will meet your academic advisor and register for your first quarter’s classes. Sign up to save your seat at an upcoming advising and orientation session.

Social Security Card

A copy of the social security card is needed when we already have the student’s information in our Student Database, but the social security number listed on the application does not match the one in the system.

Spring Quarter Grades

Student Account Hold

There is a hold on your student account. Please call the Office of Admissions at (425) 352-5000 for more information.

Submit a copy of your passport ID Page

To ensure that your UW Bothell I-20 lists your name correctly, we require a copy of your passport ID page. Please make sure that your passport is valid for at least the next six months. To learn more about the I-20 form, click here.

Submit requested Course Syllabus

A syllabus is required to determine applicability of a course toward program prerequisites.

Submit your Completed I-20 Transfer Form

Information gathered through this form will ensure your I-20 is transferred to UW Bothell. You must have an International Student Advisor at your current school complete their section of this form and begin the transfer process in the SEVIS system. To learn more about the I-20 form, click here.

Submit your Enrollment Confirmation Deposit

An enrollment confirmation of $100 is required to secure your space at UW Bothell. This $100 is credited to your account and can be applied toward future payments. Submit your enrollment confirmation deposit online here.

Submit your Final College Transcript

A final official transcript is required for verification of courses completed after the point of application. The transcript must be official to transfer credits to your UW records. To learn more about our transcript policies and how to send us your transcripts, click here.

Submit your Final High School Transcript

An official high school transcript including a graduation date. To learn more about our transcript policies and how to send us your transcripts, click here.

Submit your Financial Documentation

U.S. immigration laws require students on an F or J student visa to provide proof of adequate financial support. The financial ability requirement is met by showing proof of acceptable financial resources to cover tuition and fees, living expense, mandatory UW Health Insurance, and other course and campus fees for one academic year. Submit your Measles Immunization Form

All enrolled matriculated students admitted for Winter 2019, Spring 2019, and Summer 2019, are required to provide proof of measles (rubeola) immunity. For more information about measles immunization and to download the appropriate form, click here.

TOEFL or IELTS Score

An official TOEFL or IELTS score is required for English Language Proficiency. For more information regarding English language proficiency, click here.

Transcript Request: Fall Quarter

An updated transcript with the listed quarter’s grades must be provided for your application’s review. To learn more about sending your transcripts to the Office of Admissions, click here.

Transcript Request: Spring Quarter

An updated transcript with the listed quarter’s grades must be provided for your application’s review. To learn more about sending your transcripts to the Office of Admissions, click here.

Transcript Request: Summer Quarter

Transcript Request: Winter Quarter

Updated Personal Statement

An updated personal statement is required. This may be due to a change of requested major, or it may be requested by the program. You may email it to us at uwbinfo@uw.edu or upload it on your admission status page. To learn more about the personal statement, click here.

Verification of Enrollment in prerequisite course(s)

A copy of your in-progress course(s) registration is required to confirm that you will complete program prerequisite course(s) prior to enrollment at UW Bothell. Any offer of admission is contingent on successful completion of the course(s).

WEST-B Scores

Elementary Education Teacher Certification only. A passing score on the WEST-B must be provided. More information about the WEST-B test and options to fulfill this requirement can be found on the School of Educational Studies website.

Writing Skills Assessment Score

Business only. A Writing Skills Assessment (WSA) score is required to complete your application. If you have already registered or taken the WSA, scores will automatically be sent to the Office of Admissions. If you have not yet registered for the WSA, you may do so on the School of Business website.
