Go to the Campus Public Safety office in LB2 005. You will be required to show legal photo identification to obtain a card.
The student ID card is a permanent one. A few weeks before each quarter, after registration for classes, a Registration Confirmation containing an enrollment validation will be mailed to you.
Carry your UW ID Card with you at all times since it is required for a variety of services on campus. In some situations, you may be asked to provide additional pieces of identification, including one containing your photo.
The ID card is the property of the University of Washington and is not transferable. Do not allow others to use your ID card. Doing so is a serious misuse of the card and as such subjects you to disciplinary action as described in the University of Washington Student Conduct Code.
Some campus facilities may require you to hand over your ID card in order for you to use their facilities or equipment. Remember to pick up your card when you leave, and make sure that the card returned to you is actually your own.
If you lose your ID card, immediately call 425-685-4120 to report it as lost. You should also notify the UW library system at 206-543-2553 to prevent anyone from using your card to check out books.
To replace your ID card, go to the Campus Public Safety at LB2 005. You will be required to show legal photo identification to obtain a replacement card. A non-refundable fee of $10 is charged for replacement. If you had a U-PASS sticker on the card you lost and want to replace the sticker, there is an additional Replacement Fee. Replacement cad fees are to be paid at the UW Bothell Cashiers Office at UW1 160- Cad should be Card and Cashiers Office should be Cashier's Office.
