I. ACADEMIC POLICIES
Philosophy: The faculty, students, and staff of the University of Washington Bothell are committed to maintaining the highest standards of academic integrity. The UWB community recognizes that when the academic integrity of a university is compromised, the quality of students' education and the value of their degrees are also compromised. Therefore, all members of the university community-faculty, students, & staff-are committed to vigorously upholding the UW Student Conduct Code, Section WAC 478-120-020-2(a).

You will be held accountable for knowing the policies.
Avoiding Academic Misconduct
1. Be familiar with the Student Code of Conduct, statements, guidelines and enforcement procedures provided by the University of Washington Seattle and University of Washington Bothell. Those who violate university rules regarding academic misconduct are subject to disciplinary sanctions, including suspension and dismissal. Ignorance is no defense.
Be honest at all times and act respectfully toward others. Do not seek unfair advantage over others by cheating, plagiarizing, fabrication or facilitation.
Cheating: Giving or receiving unauthorized assistance, or intentionally using or attempting to use unauthorized materials or information.
Plagiarism: Intentionally or unintentionally using another person's original words, ideas, or research, including material found on the Internet, in any academic exercise without properly crediting that person.
Facilitation: Helping or attempting to help another student violate any part of the University of Washington Student Conduct Code, regardless of whether such help is intentional, or failing to call attention to a known violation of the Code.
Fabrication: Creating false information or data and presenting it as fact.
3. Encourage others to behave fairly and to respect ethical academic conduct. Accept responsibility by refusing to assist in others' misconduct and discouraging others from engaging in misconduct. Recognize that you should make an effort to report clear cases of academic misconduct when you have witnessed them.
4. If you are unsure about any part of an assignment, request clarification from the instructor. Failure to understand clear instructions is no excuse for misconduct.
Make safe assumptions about academic honor
It is your responsibility to understand and follow academic standards for crediting the sources of ideas, information and phraseology; otherwise you are plagiarizing.
You may not submit the same work for more than one class unless both instructors have explicitly approved this.
You may not work with others on graded coursework of unless permitted by the instructor.
You may not discuss exams or assignments with others who might also be assigned them unless the instructor has explicitly approved it.
If your assignment is a group assignment, misconduct penalties may apply to the entire group.
Always: when in doubt, ask.
SATISFACTORY PROGRESS IN THE PROGRAM: In order to graduate from the Nursing Program, a student must earn a minimum grade of 2.0 in each required Nursing course and maintain a cumulative grade point average of 2.0 or better.
A student who earns less than a 2.0 in a required Nursing course must petition the Nursing Program Faculty to be allowed to continue in the program. All petitions must be submitted through their BSN Advisor. Before filing a petition to continue, the student must meet with their BSN Advisor and/or the Program Director to develop a plan for successful progression in the program. This plan will be submitted to the Faculty along with the petition. Petitions will be reviewed individually by the Faculty and the student will be informed of their decision. In some cases, the student may be required to drop back to part-time status as a condition of continuation. All failed Nursing courses must be repeated.
Student grade reports are monitored quarterly for potential problems. Any student whose overall GPA falls below 2.5 in a given quarter will be sent a letter of concern and asked to make an appointment with their BSN Advisor for an advising session. No petition for continuation is necessary unless GPA falls below 2.0.
Each winter quarter, the faculty meets to discuss the progress of all students in the program. A letter will be mailed to each student, indicating the evaluation of his or her progress and possibly suggestions for improvement; a copy will be kept on file in the student's advising file.
ACADEMIC PROBATION: As a University of Washington Nursing student, you are subject to the University policies regarding academic standing. If, after your first quarter at the University, your UW GPA falls below 2.0, you are placed on probation by the University. A Nursing student who goes on probation must be reviewed by the Nursing Program faculty before being allowed to begin or to continue in clinical courses. If, while on probation, you are permitted to continue in the Nursing curriculum, you must achieve at least a 2.5 GPA each quarter until you bring your cumulative grade point average at the UW back to 2.0. If you earn less than 2.5 for any one quarter while on probation, you may be dropped from the Nursing Program.
WARNING NOTICE: A warning notice may be given to a student who is doing less than satisfactory work during the quarter. A warning must be given to any student who is in danger of failing or who has less than a 2.0 grade at mid-quarter, or as soon as possible thereafter. The purpose of the notice is to notify the student of difficulty in ample time so that the problem can be rectified. You will be asked to sign the notice indicating that you have read it, but you may indicate on the notice if you disagree. Warning notices are placed in your file, but are removed upon graduation. If you receive a warning notice, be sure to have a conference with the instructor so that you have a clear view of what the problem is and how it may be handled.

INCOMPLETE: The University policy (see University Handbook Vol IV-21) for granting an "Incomplete" and the requirements for converting it to a grade are:
A. An Incomplete shall be given only when the student has been in attendance and has done satisfactory work to within two weeks of the end of the quarter and has furnished proof satisfactory to the instructor the work cannot be completed because of illness or other circumstances beyond the student's control.
B. In order to obtain credit for the course, a student must convert an Incomplete into a passing grade by the last day of the following quarter. An Incomplete grade not made up by the end of the next quarter shall be converted to the grade of 0.0. For Spring Quarter, the following quarter is considered to be Fall Quarter.
CHANGE OF GRADE: The University of Washington Bothell provides the following procedure for change of grade (see section 2 of Ch. 110 of the University of Washington Policy Directory):

Dispute over grades. There are two types of disputes over grades: one alleging a punitive motive by the faculty member, and the other alleging an inaccurate reflection of work done.
1. Punitive. If a student feels a grade has been punitive (for reasons other than racial or sexual discrimination), then he or she should act under the procedures set forth in the section on unprofessional conduct in the University of Washington Handbook.
2. Inaccurate. If a student feels a grade has been an inaccurate reflection of work done in a course, he or she should attempt to resolve the matter through discussion with the instructor. If no satisfactory resolution is reached, the student may bring the issue to the Director in writing (with a copy to the instructor) within ten days of having met with the instructor. (If the instructor is the Director, then the student should submit his or her complaint to the Dean.) The Director should consult with the instructor to determine whether the grade was appropriate. If the Director concludes the grade was fair and reasonable, the student should be so informed and the issue be concluded. If the Director concludes that the instructor's grading was arbitrary and capricious, and if the instructor declines to change the grade, the Director should appoint a panel of faculty members to evaluate the student's performance and assign a grade. The Director may also appoint a substitute to assign grades when an instructor is mentally or physically incapacitated, or has left the University.
