Your application is complete only when we have received both your application to the UW Graduate School and your application to the UW Bothell Nursing Program.

- To apply to the UW Bothell Nursing Program, download the pdf files at right, complete the required forms, and submit them to our office with required accompanying documents. If for some reason you are not able to download forms, you may contact us (see below) and ask that the forms be mailed to you.
