Why is it important to complete a Mentor Contact Report?

The Mentor Contact Report is used to track the student’s compliance with their ATP Agreement. It is very important that you complete the Contact Report after each interaction with the student whether it is in person, email, phone, etc. The comments can be a brief notation of the general conversation or reason for contact. Please also note if the student does not respond to you.

While each interaction should be recorded at the conclusion of the meeting, phone conversation, etc., the final entry must be submitted by the last day of the quarter. Mentor Contact Reports are submitted through a Google Docs form.  The final entry is due on the following dates for the 2014-2015 Academic Year.

Autumn Quarter – Friday, December 12, 2014

Winter Quarter – Friday, March 20, 2015

Spring Quarter – Friday, June 12, 2015
