This form is for community-based organizations and utilizes a Lickert scale to evaluate each service learning student working with their organization—it is a vital component of a student's assessment and an important tool for maintaining good relationships with our community partners.
Evaluate a service learning program>>
This form is divided into three sections evaluating: 1) students, 2) the UW Bothell and 3) service learning in general. It offers ample room for individualized comments and provides information about the ways in which the programs at UW Bothell intersect with the community it serves.
Submit a potential service learning opportunity>>
This form was developed by the Community-based Learning and Scholarship project as a way for students and other community members to submit the needs of local organizations with which they may be familiar. It is a scaled down version of the Community Partners Registration Form and provides information which will be used by our Community Partnership Coordinators to initiate contact with the agency.

This form provides us with important information about the goals and mission of local organizations, the types of work they would like students to complete during their service-learning experience, and important contact and logistical information. All of this information is entered into our internal database to assist us in matching students with the needs of an organization.
Fill out a Risk and Consent form>>
This information must be read and acknowledged as part of the registration process. For students under 18, a hard copy must be signed by a parent or guardian and turned into the appropriate program coordinator before a student can begin volunteering at an organization for service learning.
