BIS 495 Internship Forms
- Letter of Recommendation Ask Instructor to complete and submit to Internship Adviser in UW1 169
- Application Submit to Internship Adviser in UW1 169
- Internship Confirmation Notice Submit hard copy to Internship Adviser in UW1 169
- Learning Contract Must be signed by employer, student, instructor and Internship Adviser. Student responsible for completing contract and getting all signatures.
- Mid-term Evaluation Must be completed by student and submitted to instructor.
Note: BIS 495 is only offered winter, spring and summer quarters. Dates are subject to change.
